Donation Request

Summit Dental & Orthodontics is dedicated to giving back to charitable and philanthropic causes that improve our society. We are committed to doing so in a meaningful, intentional way.

Important Guidelines

The volume of requests we receive exceeds our resources, and we are unable to honor every request. If you are a non-profit organization seeking a contribution, please refer to the following guidelines:

Local Priority: Summit Dental & Orthodontics prioritizes non-profits whose work focuses on our immediate local community.
Sports & Extracurricular Activities: Due to the nature of our practice focused on families and the overwhelming number of opportunities for children in our local community, we receive hundreds of annual requests for sponsorships of private sporting organizations or extra-curricular activities (AAU, travel sports, dance/cheer competitions, etc.). It is impossible for our practice to financially support each request, and therefore only requests submitted on behalf of sports teams through a school can be considered at this time.
Submission Timeline: Submissions must be made two months before the event/request to ensure appropriate time for review and fulfillment according to the allowable budget for sponsorships.
Required Submission: All requests are to be submitted through the Donation Request Form below.

Decision Criteria

Summit Dental & Orthodontics will give preference to organizations that meet the following criteria:

501(c)(3) non-profit organizations

Organizations that specifically serve the Layton community

Sports teams and extracurricular activities officially sponsored through a local school
Organizations that provide a “helping hand up” to members of our community in need, such as those coping with physical or mental abuse and trauma, hunger, homelessness, etc.
Organizations that strive to better our community through arts and education, leadership training, and vocational learning
